What Are Phase 4 Clinical Trials? Post-Marketing Studies and Real-World Evidence

Once an investigational product (IP) receives FDA approval, it st not the end of the IPs development journey. Even after reaching the market many IPs continue to be studied in Phase 4 clinical trials. This 4th phase helps researchers understand the real-world performance of the IP outside of the previous highly controlled environments.
What Is a Phase 4 Clinical Trial?
After Phase 3 trials the IP is either approved or not for public use by the FDA. If approved the Phase 4 research can begin. In this instance, the IP is already available commercially and healthcare providers are able to prescribe it.
During this time, researchers will continue to collect data related to the IP’s safety and effectiveness. This allows them to collect data on the IP’s long-term use and additional patient outcomes.

These studies continue free approval in order to evaluate a much lager patient population. In prior phases there were carefully established inclusion or exclusion criteria limiting the overall study population. Once an IP is approved and reaches the market it might be used by additional populations not part of the study design. This can include older individuals, individuals with multiple conditions and medications, and add tonal diverse populations.
This is all done to further evaluate the IP in a real word population to generate Real-World Evidence (RWE). This includes how patients are using the treatment outside of the trials and any new safety concerns. This also evaluates the use against earlier studies and any new safety concerns that may arise.
Health Economics and Outcomes Research

Phase 4 clinical trials also provide researchers the opportunity to evaluate broader outcomes related to their IP. Researchers may assess how satisfied patients are with the treatment. They may also evaluate if the treatment improves daily functioning and overall quality of life by improving patient daily activities. They also investigate if it improves healthcare utilization by decrease in hospitalization, emergency visits, or reducing overall healthcare costs.
This data is increasingly important to marketing initiatives for the IP, payers, providers, and healthcare systems.
Study Design and Continued Safety Monitoring
Phase 4 studies are typically less restrictive than earlier phases. These studies are usually open-label and observational. They can be registry-based which makes participation easier for both patients and research sites.
By continuous monitoring, even after approval, researchers are able to further evaluate the IP. This allows them to observe a real-world use of the IP to identify any rare adverse effects, long-term safety concerns, or population specific risks. This is all important data doe ensuring the IP remains safe throughout its lifecycle.
Opportunities for Healthcare Providers
Phase 4 studies can broaden a practical entry point into clinical research. This is due to phase 4 clinical trials providing opportunities for provider to:
Expand research experience
Engage more patients
Contribute to evidence generation
Build relationships with sponsors
For many practices, these studies provide a practical entry point into clinical research.
